What is an incident ?
The report form is on the www.taso.org web site on the top banner, it is an online form
FILL IT OUT AND SEND IT ONLY AFTER DISCUSSING THE INCIDENT WITH THE 1ST VP AND RECEIVING HIS APPROVAL TO SEND
The following incident reports can be filed without any interaction with the 1st VP.
Examples of incidents
Example of Red Card Incident – Serious Foul Play
On January 12th, @ Plano’s Kimbrough Stadium in the Boys Varsity match, Player # 3 Wayne Whackem of McKinney Boyd in the 56th minute used excessive force and endangered the safety of opponent.
Example of Red Card Incident – Violent Conduct
On February 3rd, @ Mesquite’s Memorial Stadium in the Girls JV Match, Player # 6 Hailey Holdagrudge of Mesquite Poteet in the 72nd minute punched an opponent.
Example of Red Card Incident – Foul and Abusive Language
On February 14th, @ Garland’s HBJ Stadium in the Boys JV Match, Player # 9 Freddy Foulmouth of South Garland in the 69th minute told the ref “F*** you ref. You suck” – (Please do not paraphrase. Report what was said verbatim).
Example of Red Card Incident – 2ndCaution
On March 5th, @ Dallas ISD Forrester Stadium in the Boys Varsity Match, Player #22 Alfred Againandagain of Pinkston committed reckless challenge in the 28th minute for first caution (Unsportsmanlike Conduct). Alfred committed second reckless challenge in the 65th minute and received second caution (Unsportsmanlike Conduct).
Example of Red Card Incident – Denies Obvious Goal Scoring Opportunity (DOGSO) inside penalty area
On January 21st, @ Frisco’s Memorial Stadium in the Girls Varsity Match, Player #5 Lucy Lastdefender of Frisco Wakeland in the 22nd minute denied obvious goal scoring opportunity because of (holding, pulling, pushing) in the penalty area.

Example of Red Card Incident – Spitting on Opponent, Teammate, Official
On February 23rd, @ Carrollton Farmers Branch Standridge Stadium in the Girls JV Match, Player #99 Susie Hocker of Carrolton Newman Smith in the 12th minute spit on an opponent.
Example of Weather Related Incident
On February 28th, @ Forney’s City Bank Stadium, the Boys Varsity match was stopped in the 28th minute of the 1st half because of inclement weather (Lightning, Rain, etc)
